IMPORTANT NOTICE: Availability of Your 2025 ACA Health Coverage Tax Form (Form 1095-C / 1095-B)

Under recent updates to federal tax law, Guppy’s on the Beach, LLC, a Florida Limited Liability Company, is no longer required to automatically mail paper copies of IRS Form 1095-C to all employees and covered individuals.

Form 1095 is a tax document that includes information about the health care coverage offered to you and your dependents by Guppy’s on the Beach, LLC during the previous calendar year. While you do not need to attach this form to your federal income tax return, you may need it to help complete your tax filing.

How to Request Your Form 1095

You have the right to request a copy of your 2025 Form 1095. Upon request, we will provide your form to you within 30 days.
